Caused by imaging the elbow in hyperextension, resulting in appearance of osteochondral defect of the posterolateral capitellum. The giveaway is correlation of the location of the lesion on the coronal images revealing it to be in a far extra-articular postural natural position falling off the back of the capitellum.
